b'SPECIAL EDITION 202027Navigating Forward Fund Concord Hospital Trust created the Navigating Forward Fund to help patients with out-of-pocket expenses. As part of the Hospitals COVID-19 response, it expanded to help keep some patients out of the Hospital or doctors offices. The fund has been used in the past to keep Concord Hospital Medical Group (CHMG) patients healthy, at home, by purchasing an air conditioner for a patient with respiratory issues or a refrigerator for another to keep prescription medication cold. Now, it is helping provide medical devices for CHMG patients with chronic health conditions, so they may monitor themselves at home. Self-monitoring at home avoids trips to the Hospital or doctors office, reducing any potential COVID-19 exposure for high-risk patients and ensuring medical resources are available for COVID-19 patients or others who need Hospital care. The fund has provided devices including blood pressure cuffs, glucometers to measure blood sugar, scales to keep track of weight changes and monitors to track blood-oxygen levels. Information from home monitoring devices help healthcare providers keep track of their patients chronic conditions, including diabetes, heart failure and respiratory ailments.CHMG patients who need medical devices for home monitoring should contact their CHMG Nurse Navigator.
